Registration Procedures
1. The registration procedures for new college student are:Registrar:
The student should submit the necessary general admission requirement stated earlier.
Steps:- 1. The student goes to the Dean for an interview and the latter will sign the confirmation slip.
- 2. The student goes to the registrar's office and gets an enrollment form. Concerned applicant submit NSTP form to the commandant/Registrar.
- 3. Registrar signs enrollment form and issues permit for picture taking.
- 4. Finance Office:
- a. Assessment of tuition and other fees
- b. Payment of tuition and other fees
- 5. Registrar issues class cards.
- 6. Student submits the filled class card to the registrar.

| Design | A-line deep pleats, two in font, two at the back. Then should be two inches. Waistband 1½ inches. Side secret pockets. Opening is at the left side with zipper. |

| Design | Long sleeves, pointed collar with ribbon (approx.1 cm in width, 28 inches long – material is the same with the skirt) |
| Length | Approximately 4 inches below the waistline. |
| Shoes | Black (closed) with 1-2 inches heel. Sandals, slippers, and sling backs are not allowed. |
| For Male Students | Polo shirt |
| Material | Cotton (Plain white) with collar. |
| For Male Students | Pants |
| Material | Any dark colored pants is allowed as long as it is not tattered. |
| Shoes | Any closed shoes. Sandals, clogs and slippers are not allowed. |
2. The registration procedure for old enrollees is as follows:
- 1. Proceed to the Guidance office and fill up a student Inventory form.
- 2. Secure Pre-enrollment form from the registrar's Office. For male enrollees secure NSTP enrollment form.
- 3. Determine your subject load and have it approval by the Registrar.
- 4. Secure an Official Enrollment form and copy approved subject load neatly, legibly and accurately.
- 5. Proceed to the Treasurer's Office for the assessment for the fees and payment of tuition and other fees.
- 6. Proceed to the Registrar Office for the issuance of class card.
- 7. Fill up class card neatly, legibly, and accurately. Changing of subject/schedule is not allowed.
- 8. Submit your filled up class card to the Registrar Office.
Note: Applicant can secure class card by presenting his/her receipt of payment at the Treasurer Office. Every class card must bear the stamp. "Officially Enrolled". Form must be filled up correctly, neatly and legibly. Dirty forms and class cards have to be changed and paid for.
3. During prescribe date/period, a student may, with the approval of the Dean concerned, changes subject(s) by filling up a prescribed form. All changes must be reported to the Registrar and the Treasurer for proper adjustment.
